Telehealth services offered through Ellory Health are managed and coordinated by Ola Digital Health, https://oladigital.health/

• Medical evaluations, prescribing decisions, and ongoing care are provided by licensed medical providers coordinated through WellSync.
• TrimMD Wellness Center LLC d/b/a Ellory Health does not provide medical care or medical evaluations.
• TrimMD Wellness Center LLC d/b/a Ellory Health does not employ clinicians or licensed medical providers.

2. Where Telehealth Services Are Available

Telehealth services are available only in jurisdictions where licensed medical providers are authorized to practice and where services are clinically appropriate under applicable law.

a. Asynchronous Telehealth States

In certain states, telehealth services may be provided asynchronously in accordance with applicable state law. This may include completion of an online health questionnaire that is reviewed by a licensed medical provider.

Residents of the following states may be eligible for asynchronous telehealth services:

b. Synchronous Telehealth States

In other states, a real-time virtual visit (audio and/or video) with a licensed medical provider is required before any treatment determination or prescribing decision can be made.

Residents of the following states require a synchronous telehealth visit:

AK, AR, CO, DE, HI, ID, KS, LA, ME, MS, MO, NM, ND, RI, UT, WA, WV

3. Additional Disclosures

• Telehealth availability, modality (asynchronous or synchronous), and treatment decisions are determined solely by licensed medical providers in accordance with state and federal regulations and clinical appropriateness.
• Not all services are available in all jurisdictions.
• Eligibility for treatment is subject to provider review and medical discretion.
• Prescriptions, if issued, are based on a provider’s professional medical judgment.
